The model

Pharmaceutical care can be defined as "the professional practice through which the pharmacist engages with the patient (and/or caregiver) and other health providers to cater for the former’s pharmaceutical needs, implementing the strategies required to achieve the short- and long-term goals of their therapy and embracing new technologies and any other means required to establish a continuous interaction with them so as to improve their health outcomes."
