Capacity
"Capacity" is defined as "the provision of pharmaceutical care taking into account the patient’s individual needs.” This requires using the existing stratification models, which are based on a series of variables, grouped into dimensions, which have an impact on the extent to which patients are able to achieve the health outcomes related to their specific condition.
Use of these models makes it possible to modulate the intensity of pharmaceutical care and ensure a more structured patient follow-up.
